Overview

Incident Investigation is a high-value corporate programme designed to help organisations learn effectively from incidents, near misses, unsafe conditions, and process deviations—so recurrence is prevented and risk controls are strengthened. A poor investigation focuses on blame and symptoms; an effective investigation focuses on facts, evidence integrity, causal analysis, and system improvement.

This course equips supervisors, HSE teams, engineers, and managers with a structured investigation method to identify immediate causes, contributing factors, and underlying organisational weaknesses—then convert findings into robust corrective and preventive actions (CAPA) that deliver measurable risk reduction. The programme is suitable for all industries and can be tailored for construction, manufacturing, oil and gas, logistics, healthcare, laboratories, and corporate workplaces.

What You’ll Learn?
Understand the purpose of incident investigation and the difference between learning-focused investigations and blame-focused reporting.
Classify events correctly (incident, near miss, unsafe condition, dangerous occurrence) and apply an appropriate investigation depth.
Secure the scene safely and preserve evidence while managing emergency response and operational continuity.
Collect high-quality evidence: witness interviews, photos, sketches, documents, permits, maintenance records, training records, and CCTV (where available).
Conduct professional interviews using non-leading questions and supportive communication that improves accuracy and trust.
Apply structured analysis tools: 5 Whys, Fishbone (Ishikawa), causal factor charting, barrier/critical control analysis, and basic root cause logic.
Identify organisational and management system weaknesses (competence, supervision, planning, contractor control, change management, maintenance, design, and culture).
Develop effective corrective actions using hierarchy of controls—moving beyond training and reminders to engineering and system improvements.
Write clear investigation reports and present findings to management with strong evidence and practical recommendations.
Track actions to closure and verify effectiveness through follow-up monitoring and learning loops.
